Fredericton Downtown Community Health Centre

In 2014, Horizon and the University of New Brunswick (UNB) Community Health Clinic formed a partnership to create the Fredericton Downtown Community Health Centre (FDCHC). This is a teaching facility that delivers an integrated program in which primary health care services, teaching and research are combined. Horizon and the UNB faculty of nursing are integrated into this community health centre, providing opportunities for students to gain experiential learning and to advance research and innovations in community-based health care. In March 2017, the FDCHC officially opened their new space at 339 King Street, Fredericton. FDCHC provides services including illness and injury prevention, chronic disease management, community development and outreach services, using a population health promotion approach. It is made up of a team of health providers who work together in a collaborative practice with community participation and a focus on patient-centred care. Health services include scheduled and unscheduled appointments with a health care provider, as well as a wide range of diagnostic and support services from the health care team. A unique feature of FDCHC is its Community Access Room, a laundry and showering facility for the vulnerably housed or homeless population.
