Queens North Community Health Centre

The Queens North Community Health Centre (QNCHC) is a facility that provides primary health care services, illness/injury prevention, chronic disease management and community development services, using a population health promotion approach. QNCHC is made up of a team of health care professionals who are all about teamwork, collaborative practice, community participation, and patient-centred care. The team includes physicians, nurse practitioners, social workers, dietitians, health promoters, counselors and other health care providers.

The Queens North Community Health Centre offers a range of health services, including scheduled and unscheduled appointments with a physician or nurse practitioner, as well as a wide range of diagnostic and support services from the health care team.

Some of the programs happening in the Grand Lake area:

  • Taste Buds – introducing children and their parents to healthy foods
  • CHEER – a physical activity and health promotion program designed to engage children in the pleasures and benefits of physical activity and healthy living
  • Off to school programs
  • Harvest Fest – community event promoting healthy lifestyles and CHC/Community programs and services
  • Food drive for the food bank
  • Caregiver support groups
  • Diabetic information sessions – now using education as per diabetic mapping
  • Diabetic cooking course
  • Grit bags – made available to the community for fall prevention
  • Resistance band training – to help people improve their strength and muscle tone
  • Get Mov’n Minto / Mountain Bike Minto
  • NEW program – a 12-week nutrition and exercise program
  • Introduction to solids program
  • Cooking courses
  • Drop in at the Care N’ Share for breast feeding support offered by the CHC
  • Footsteps program with spring and fall fun runs offered through the wellness network
  • Food mentor program
  • Food smart program with 200 bags monthly, supported by volunteers
  • Breakfast program – at the schools, supported by CHC Health fairs
  • Leisure activity in schools
