During these outreach hours, individuals can access a variety of services including assessment, support, education, treatment for substance use, Hepatitis C screening, naloxone kits, nicotine replacement therapy, and basic wound care. We also offer case management, patient navigation, and support with connecting to community resources and ongoing care.
