The Hepatitis C team at SCHC is part of the Ontario Hepatitis C Network. We aim to provide prevention and care services for those at risk or living with Hepatitis C. We operate in the community providing point-of-care testing, education, linkage-to-care and treatment, and mental health support with a wrap-around care model.

This work is done with an interdisciplinary team of a Nurse practitioner, Harm Reduction worker, Ambassador, Social Worker, and Medical Secretary.
We are committed to working with the community and those with lived experience to ensure a client-centred approach to care.
