The Tobique Valley Community Health Centre (TVCHC) is an organization that provides primary health care services, illness/injury prevention, chronic disease management and community development services, using a population health promotion approach. TVCHC is made up of a team of health care professionals who are all about teamwork, collaborative practice, community participation, and patient centred care. The team includes physicians, nurse practitioners, social workers, dietitians, health promoters, counselors and other health care providers.
