SCHC provides comprehensive, focused health programs and services to improve the holistic overall health and well being for our community.

The Hepatitis C team at SCHC is part of the Ontario Hepatitis C Network. We aim to provide prevention and care services for those at risk or living with Hepatitis C. We operate in the community providing point-of-care testing, education, linkage-to-care and treatment, and mental health support with a wrap-around care model.This work is done with an interdisciplinary team of a Nurse practitioner, Harm Reduction worker, Ambassador, Social Worker, and Medical Secretary.
